Our Perfect Match Promise:

If following your initial consultation you didn’t connect with your practitioner, please email us at info@sportspodiatrists.com.au within 60 days of the consultation and we will assist you to find another suitable appointment with a different practitioner. In those instances, Sports Podiatrists Pty Ltd agrees to cover the cost of one session with a new practitioner of equal value to the prior consultation i.e. either a F004 (Initial) or F014 (Standard) appointment for 60mins (Sports Podiatry) or 30 mins (General Podiatry). The Perfect Match Promise consultation may only be claimed once per patient and cannot be applied again after claiming one Perfect Match Promise consultation. Standard cancellation policy applies: Cancelling within 24 hours or failing to attend will result in the complimentary session being forfeited. Retrospective refunds will not be granted for past sessions. If you have attended Sydney West Sports Medicine, Sydney SportsMed Specialists or St Vincent’s SportsMed you will be required to travel to one of our other clinics for your complimentary consultation. Feedback and comments will be kept confidential.

PODIATRIC SERVICES
As part of providing our podiatric service to you it is necessary to collect and record personal information at the clinic where you see your podiatrist. The information gathered is part of our assessment, diagnosis and treatment. It enables your podiatrist to provide a relevant and informed podiatric service.

Confidentiality
All personal information gathered by the clinic and podiatrist during the provision of the podiatric service will remain confidential and secure except where:

1. It is subpoenaed by a court.

2. Failure to disclose the information would place you or another person at serious and imminent risk.

There may be times where, as part of the assessment and management process, it may be helpful for your podiatrist to communicate with and /or write to other health practitioners that are relevant to your goals. Additional consent will be requested in these instances.

Please note that if you intend to claim rebates from Medicare or your health fund, or if your injury is under Workers Compensation or Australian Defense Force, your podiatrist may be required to provide summary reports to external agencies regarding their treatment progress. Under the Medicare scheme these reports are typically be sent to your GP and any other relevant health practitioner i.e. your Physiotherapist.

Consultation and Orthotic Fees
The cost of our initial and standard consultations fees and orthotic therapy are available here and any additional costs will be discussed and set out before treatment commences. Please note the prescription of customised orthotics requires a F117 (assessment) and F303 (3D foot scan) to be charged initially. All fees are payable at the end of the consultation. Payment can be made by cash or Eftpos and can be used in conjunction with HICAPs health fund rebate where available. If your health fund does not work, or does not cover some or all of the items, you will be required to pay the full gap amount.

In the event that you are not happy with your orthotics, we will happily refund you the gap payment on your orthotic costs (F221).

Cancellation Policy
Please respect that we have waitlists for our appointments and although we understand that unforeseen and unavoidable circumstances arise, please provide as much notice as possible so we can allocate that appointment to someone else. If for some reason you need to cancel or postpone your appointment, at least 24 hours notice is appreciated. A cancellation fee of 100% may be charged if insufficient notice is given (i.e. notice within 24 hours). All cancellations must be made via phone to the relevant clinic. e-mail cancellations will not be accepted.  If you fail to attend an appointment, with no notice of cancellation, the full fee will apply.

If numerous consecutive cancellations are made, then it is also at our discretion to charge a non-refundable deposit for future consultations. Clients with a pattern of late cancellations and non-attendance may have their appointment slot reallocated to another client on the waitlist or their frequency of consultation reduced.

Conditions of Cancellation Policy
If a cancellation fee has been charged, it must be paid prior to the provision of any further services. These cancellation fees apply to all clients of Sports Podiatrists even those whose services are paid for by another organisation (Medicare, Private Health funds, etc). The cancellation fee will need to be paid personally by the client as third party organisations do not typically cover cancellation fees for nonattendance at scheduled appointments.

Tardiness

We aim to run on time for each and every one of our consultations and rely on the tardiness of our patients to help us stay on time. Our consultation times are strictly allocated to either half hour or one hour and will not be extended past the consultation time due to late arrivals. Full fees apply in circumstances of late arrival.
